Recognize the Warning Signs

Warning Signs of Self-Harm

Self-harm can take many forms. Understanding the warning signs is the first step toward getting help for yourself or someone you care about.

Behavioral Warning Signs

  • Wearing long sleeves or pants even in warm weather to hide injuries
  • Withdrawing from friends, family, and social activities
  • Making excuses for frequent cuts, burns, or bruises
  • Keeping sharp objects or first-aid supplies hidden
  • Spending long periods alone, especially in bathrooms or bedrooms

Physical Signs

  • Unexplained cuts, scratches, burns, or bruises — often in patterns
  • Evidence of hair pulling (thinning patches, bald spots)
  • Skin picking that leaves wounds or scarring
  • Hitting or banging oneself against surfaces
  • Wounds that don’t heal or keep reappearing in the same areas

Emotional & Psychological Signs

  • Expressions of hopelessness, worthlessness, or feeling trapped
  • Difficulty managing emotions — intense anger, sadness, or numbness
  • Talking about feeling out of control or needing to “feel something”
  • Increased irritability or emotional instability
  • Describing self-harm as a way to cope, punish oneself, or release pain

If you recognize any of these signs in yourself or a loved one, compassionate help is available.

Understanding Self-Harm

Self-Harm by the Numbers

Self-harm is more common than many people realize. Research indicates that approximately 17% of adolescents and young adults report engaging in self-harm at some point — making it one of the most prevalent mental health concerns in younger populations.

Self-harm is not limited to any single demographic. It affects people of all ages, genders, and backgrounds, often co-occurring with depression, anxiety, PTSD, and borderline personality disorder. In New Hampshire, the intersection of mental health challenges and limited access to specialized care means many individuals go without the treatment they need.

These numbers represent real people in Salem, Derry, Nashua, Manchester, and across the region who deserve compassionate, evidence-based care — not judgment.

How to Help a Loved One Who Self-Harms

Stay Calm and Listen Without Judgment

Discovering that someone you love is self-harming can be frightening. Your instinct may be to react with shock or anger, but the most helpful response is to stay calm and listen. Let them know you care without making them feel ashamed. Avoid saying things like “just stop” — self-harm serves a purpose for them, and they need help finding healthier alternatives.

Educate Yourself About Self-Harm

Self-harm is a coping mechanism, not a cry for attention. People who self-harm are often trying to manage overwhelming emotions, feel something when they feel numb, or express pain they can’t put into words. Understanding this helps you respond with compassion rather than fear.

Don’t Issue Ultimatums

Demanding that someone stop self-harming rarely works and can drive the behavior underground. Instead, focus on the underlying emotions. Encourage them to talk to a professional and offer to help them find treatment.

Encourage Professional Help

Self-harm often requires professional treatment, especially Dialectical Behavior Therapy (DBT), which teaches healthier ways to cope with intense emotions. Offer to call Trailhead together, or drive them to their first appointment.

Take Care of Yourself

Supporting someone who self-harms can be emotionally exhausting. Your mental health matters too. Consider therapy for yourself, set healthy boundaries, and know that you are not responsible for “fixing” them — that is the work of trained professionals.

When a Loved One Won’t Seek Help

What If They Won’t Accept Help?

It’s common for people who self-harm to resist treatment. This doesn’t mean recovery is impossible — it means a different approach may be needed.

Don’t let shame or discomfort shut down conversations. Let them know you’re available to talk whenever they’re ready — without pressure. Sometimes just knowing someone cares and won’t judge them is the first step toward accepting help.
Use “I” statements: “I’m worried about you” rather than “You need to stop doing this.” Focus on your care for them, not the behavior itself. Confrontation often triggers shame and withdrawal.
Full treatment can feel overwhelming. Suggest they call a helpline, talk to their primary care doctor, or attend one session. Small steps reduce resistance and build trust in the process.
If safe to do so, reduce access to items used for self-harm. This is a harm-reduction strategy — not a cure — but it can reduce the severity and frequency of episodes while they work toward treatment.
If there’s immediate danger — suicidal thoughts, severe self-injury requiring medical attention, or risk of serious harm — call 911 or the 988 Suicide & Crisis Lifeline. Our admissions team can also guide you through next steps at (857) 312-1697.

Frequently Asked Questions

Self-Harm Treatment FAQ

Not necessarily. Self-harm and suicidal behavior are different — self-harm is often a coping mechanism to manage overwhelming emotions, not an attempt to end one’s life. However, people who self-harm are at elevated risk for suicidal thoughts and behaviors, which is why professional treatment is so important.
Dialectical Behavior Therapy (DBT) is the gold standard treatment for self-harm. It was originally developed specifically for people who struggle with self-harming behaviors and emotional dysregulation. DBT teaches four core skill sets: mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness — giving you practical tools to replace self-harm with healthier coping strategies.
A safety plan is a personalized, written strategy developed with your therapist that outlines your triggers, warning signs, coping strategies, people you can contact for support, and steps to take during a crisis. It serves as a concrete guide to help you navigate difficult moments without turning to self-harm.
PHP typically lasts 20–30 days, IOP runs 60–90 days, and ongoing outpatient support continues as needed. Most clients benefit from a step-down approach from higher to lower intensity as they build coping skills and stability.
Absolutely. Our Evening Professional Track (6–9 PM, Mon-Thu) is designed for working adults. We also offer afternoon IOP and telehealth options with flexible daily scheduling.
Most major plans cover mental health treatment. We’re in-network with Anthem BCBS, Point 32 Health, Tricare, Uprise Health, and WellSense. Our team verifies benefits in minutes.
Family involvement is a critical part of self-harm recovery. We offer individual family therapy sessions plus monthly Zoom education groups covering how to recognize warning signs, communicate supportively, and create a safe home environment. Open to any family member 18+.
It depends on your individual needs. Our on-site Nurse Practitioner may prescribe antidepressants, mood stabilizers, or anti-anxiety medications to address underlying conditions like depression, anxiety, or PTSD that often co-occur with self-harm. Medication is always combined with therapy.
We plan aftercare from day one: step-down to IOP or OP, alumni program, external therapist referrals, continued family support groups, and an updated safety plan for ongoing recovery.
DBT-based programming (the gold standard for self-harm), individualized safety planning, a compassionate non-judgmental environment, flexible scheduling, 12:1 counselor caseloads, and same-day admission when clinically appropriate.
