What If They Refuse Treatment?

It’s common for people living with bipolar disorder to resist treatment — especially during manic episodes when they feel invincible. This doesn’t mean recovery is impossible — it means a different approach may be needed.

During mania, people often feel great and don’t believe anything is wrong. During depression, they may feel too hopeless or exhausted to seek help. Anosognosia — a lack of awareness of their own condition — affects up to 50% of people with bipolar disorder. Understanding this helps you respond with compassion.
Conversations about treatment are most effective during euthymic (stable) periods. Use “I” statements and express concern without judgment. Discuss what you’ve observed and how it impacts the family. Avoid confrontation during active mood episodes.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept — especially around dangerous manic behaviors — and maintain those limits consistently.
