Clinical Foundation

Every IOP client receives weekly individual therapy paired with evidence-based group sessions. Our clinical team draws from CBT, DBT, and ACT to build a framework of skills you'll use long after treatment ends. A Nurse Practitioner meets with you within 72 hours of admission for medication evaluation and management.

Choose the schedule track that fits your life: Morning (9:00am–12:30pm), Afternoon (12:30–3:30pm), or our Professional Evening Track (6:00–9:00pm, Mon–Thu). Each 3.5-hour session includes group therapy, psychoeducation, and case management — with snacks provided during sessions.

Community & Connection

Recovery doesn't happen in isolation. Our IOP builds connection through fellowship time and sober activities outside the clinic. Clients participate in equine therapy at Blue Sky Farm, group hikes, bowling outings, and local recovery events — building a sober support network that extends beyond these walls.

Family members are part of the process too. Monthly Zoom support groups, individual family therapy sessions, and education on codependency and family systems help your loved ones understand addiction and support your recovery. Family groups are open to any family member 18+, including families of past clients.

Four Phases of IOP Treatment
Phase One

Assessment & Orientation

Your IOP journey begins with a comprehensive clinical assessment. Together with your assigned therapist and case manager, you'll develop a personalized treatment plan that addresses both substance use and any co-occurring mental health conditions. You'll choose your schedule track — Morning, Afternoon, or Evening — and meet with our Nurse Practitioner within 72 hours for medication evaluation and management. This phase is about finding your footing and mapping your path forward.

Phase Two

Skill Building & Curriculum

With your treatment plan in place, you'll move into the heart of the program. Group sessions build your toolkit — CBT helps you identify and reframe negative thought patterns, DBT teaches emotion regulation and distress tolerance, and ACT helps you develop psychological flexibility. Weekly individual therapy goes deeper into your personal story. Psychoeducation groups give you the knowledge to understand what's happening in your brain and body during recovery.

Your 3.5-hour sessions are structured around group therapy, psychoeducation, and skills practice. Between sessions, you're applying what you've learned in real life — at work, at home, in relationships. That immediate application is what makes IOP so effective.

Phase Three

Real-World Application

As your confidence grows, the focus shifts from learning skills to mastering them in everyday situations. You're managing stress at work, navigating family dynamics, and building healthy routines — all while attending IOP sessions that reinforce your progress. Sober activities like equine therapy, hiking, and community events help you discover that life in recovery can be genuinely fulfilling. Family involvement deepens through therapy sessions and monthly Zoom support groups.

Phase Four

Transition & Continuing Care

Before you complete IOP, we build a bridge to what comes next. Your care team coordinates your step-down to our standard Outpatient Program, connects you with our alumni network, and sets up any external referrals you may need. If sober living is part of your plan, we help arrange that too. You'll leave with a clear aftercare plan, ongoing support, and the knowledge that Trailhead's community remains open to you.

Intake & Orientation

You'll be assigned a primary therapist and case manager, take a facility tour, and meet your fellow clients. We handle the logistics so you can focus on showing up.

PHP is more intensive: 9am–3:30pm (full day) with daily lunch provided and NP access within 24 hours. IOP offers 3.5-hour sessions with three schedule tracks, NP access within 72 hours, and a longer overall duration (60–90 days vs. 20–30). Many clients start at PHP and step down to IOP as they stabilize. IOP provides more flexibility while maintaining strong clinical support.

Substance Use Disorders

Our IOP treats a full range of substance use disorders including alcohol, opioids, heroin, fentanyl, cocaine, methamphetamine, benzodiazepines, marijuana, kratom, and prescription drug abuse. We also address less common dependencies including synthetic drugs, hallucinogens, inhalants, stimulants, barbiturates, and MDMA. Whatever the substance, our clinical framework adapts to your specific needs.

Co-Occurring Mental Health

Many clients come to us with more than just a substance use disorder. We treat co-occurring anxiety, depression, PTSD, bipolar disorder, OCD, panic disorders, trauma, and self-harm behaviors.
