Intensive daily treatment from 9am–3:30pm, Monday through Friday. Structured care that keeps you grounded while you heal — in Salem, New Hampshire.
“The staff genuinely cares. The groups were powerful, the individual therapy was life-changing, and I never felt like just a number. Trailhead gave me my life back.”
Our PHP is built around three core pillars. Each reinforces the others, creating a treatment experience that addresses the whole person — not just the addiction.
Every client receives weekly individual therapy paired with daily evidence-based group sessions. Our clinical team draws from CBT, DBT, and ACT to build a framework of skills you'll use long after treatment ends. A Nurse Practitioner meets with you within 24 hours of admission for medication evaluation and management.
Your day runs from 9am to 3:30pm, structured around group therapy, individual sessions, psychoeducation, and case management — with lunch provided to keep fellowship time built into the rhythm of care.
Recovery doesn't happen in isolation. Our PHP builds connection through shared meals, fellowship time, and sober activities outside the clinic. Clients participate in equine therapy at Blue Sky Farm, group hikes, bowling outings, and local recovery events — building a sober support network that extends beyond these walls.
Family members are part of the process too. Monthly Zoom support groups, individual family therapy sessions, and education on codependency and family systems help your loved ones understand addiction and support your recovery.
PHP isn't just about getting stable — it's about building a life worth staying sober for. Case management support covers resume building, job searching, court paperwork, and daily life skills. We work with you to apply therapeutic skills in real-world situations before you step down to IOP.
When you're ready, we coordinate your transition to our Intensive Outpatient Program, alumni network, and community resources. No one leaves Trailhead without a clear plan forward.
Structure your recovery around your life. Attend treatment 9am–3:30pm and return home or to sober living each evening.
Mix morning, afternoon, or evening sessions to fit work, family, or other obligations. We adapt to your lifestyle.
Medication evaluation and management starts immediately — no waiting weeks to see a prescriber.
Shared meals create fellowship and community. Breaking bread together is part of the healing process.
Dialectical Behavior Therapy forms the core of our group curriculum, building emotion regulation and distress tolerance skills.
Regardless of program level, you get one-on-one time with a licensed therapist every single week.
Choose from CBT, DBT, ACT, 12-Step, SMART Recovery, or Dharma Recovery — your recovery, your approach.

Your journey begins with a comprehensive clinical assessment and a meeting with our Nurse Practitioner within your first 24 hours. Together with your assigned therapist and case manager, you'll develop a personalized treatment plan that addresses both substance use and any co-occurring mental health conditions. This phase is about getting grounded — understanding where you are so we can map where you're going.
With your treatment plan in place, you'll move into the heart of the program. Daily group sessions build your toolkit — CBT helps you identify and reframe negative thought patterns, DBT teaches emotion regulation and distress tolerance, and ACT helps you develop psychological flexibility. Weekly individual therapy goes deeper into your personal story. Between sessions, psychoeducation groups give you the knowledge to understand what's happening in your brain and body during recovery.
Your daily rhythm settles into a structured pattern: morning groups followed by individual work, a shared lunch that builds community, then afternoon psychoeducation and case management before heading home at 3:30pm. It's intensive but sustainable — designed to fill your day with purpose without burning you out.
As your confidence grows, the focus shifts from learning skills to applying them. You'll practice new coping strategies in real-life situations — managing stress at home, navigating relationships, and building healthy routines. Sober activities like equine therapy, hiking, and community events help you discover that life in recovery can be genuinely fulfilling. Family involvement deepens through therapy sessions and monthly support groups.
Before you leave PHP, we build a bridge to what comes next. Your care team coordinates your transition to IOP (morning, afternoon, or evening tracks available), connects you with our alumni network, and sets up any external referrals you may need. If sober living is part of your plan, we help arrange that too. You'll leave with a clear aftercare plan, ongoing support, and the knowledge that Trailhead's community remains open to you.

Verify Your CoverageOur PHP treats a full range of substance use disorders including alcohol, opioids, heroin, fentanyl, cocaine, methamphetamine, benzodiazepines, marijuana, kratom, and prescription drug abuse. We also address less common dependencies including synthetic drugs, hallucinogens, inhalants, stimulants, barbiturates, and MDMA. Whatever the substance, our clinical framework adapts to your specific needs.
Many clients come to us with more than just a substance use disorder. We treat co-occurring anxiety, depression, PTSD, bipolar disorder, OCD, panic disorders, trauma, and self-harm behaviors. Our dual-diagnosis approach ensures both conditions are addressed simultaneously — because treating one without the other rarely leads to lasting recovery.
