The Reality in New Hampshire

Heroin Use in New Hampshire

Heroin-related deaths in New Hampshire remain a significant concern despite fentanyl becoming more prevalent in the drug supply. Many heroin users in the state transitioned from prescription opioid misuse, following a well-documented pipeline from pills to injection drug use.

New Hampshire has been one of the states hardest hit by the opioid epidemic. In response, the state has expanded access to naloxone (Narcan) to combat heroin and opioid overdoses, making it available at pharmacies without a prescription and distributing it through community outreach programs.

These aren’t just statistics — they represent families in Salem, Derry, Nashua, Manchester, and across the region who need access to compassionate, evidence-based heroin addiction treatment.

For Family Members

How to Help a Loved One with Heroin Addiction

Understand the Overdose Risk

Heroin addiction carries an immediate risk of fatal overdose, especially with fentanyl-contaminated supply. Learn the signs of overdose: blue lips, shallow breathing, unresponsiveness. Keep naloxone (Narcan) accessible — it can reverse an opioid overdose and save a life.

Learn About Naloxone (Narcan)

Naloxone is available at NH pharmacies without a prescription. Keep it in your home, car, and bag. Learning to administer it takes minutes and could mean the difference between life and death. Ask your pharmacist or call Trailhead for guidance.

Stop Enabling, Start Supporting

There’s a difference between supporting someone and enabling their heroin use. Stop giving money, making excuses, or shielding them from consequences. Enabling delays the moment they recognize they need help. Express concern honestly and offer treatment as a solution.

Consider an Intervention

Because heroin addiction escalates quickly and overdose risk is high, waiting for them to “hit bottom” can be fatal. A professional interventionist can help your family create a structured, compassionate plan to motivate your loved one to accept treatment before it’s too late.

Practice Needle Safety

If your loved one is actively using, reduce harm where possible. Dispose of needles safely, avoid sharing injection equipment, and encourage them to use clean supplies. Harm reduction saves lives and keeps the door open to recovery.

Family Support at Trailhead

Our family support program is open to all family members 18+, past and present clients. We offer individual family therapy sessions and monthly Zoom education groups covering codependency, family systems, and opioid addiction education.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with heroin addiction to resist treatment. With the risk of fatal overdose, time is critical — but there are effective approaches even when someone isn’t ready.

With heroin, the stakes are life-or-death. A structured, guided intervention led by a professional can break through denial and help your loved one see the impact of their heroin use on themselves and the people they care about. Because overdose risk is high, acting sooner rather than later is critical.
Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change — helping them arrive at the decision to seek help on their own terms.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept, and maintain those limits consistently.
People move through precontemplation, contemplation, preparation, action, and maintenance. Meeting them where they are is more effective than pushing. Recognizing which stage they’re in helps you respond appropriately.
If there’s immediate danger — overdose symptoms (blue lips, shallow breathing, unresponsiveness), suicidal thoughts, or severe withdrawal — call 911 immediately and administer naloxone (Narcan) if available. Our admissions team can also guide you through next steps and discuss intervention options.

Frequently Asked Questions

Heroin Addiction Treatment FAQ

Heroin withdrawal is intensely uncomfortable but rarely life-threatening. Symptoms include muscle and bone pain, vomiting, diarrhea, cold flashes, insomnia, and intense cravings. Withdrawal typically peaks 24–72 hours after last use and lasts 5–10 days. MAT medications like Suboxone can dramatically reduce withdrawal severity.
Suboxone (buprenorphine/naloxone) is a partial opioid agonist that reduces cravings and withdrawal symptoms without producing the euphoric high of heroin. It’s taken as a sublingual film or tablet and is one of the most effective treatments for opioid use disorder. Our NP manages all Suboxone prescriptions on-site.
Both are effective. Suboxone has a lower abuse potential and can be prescribed in-office, while Methadone requires daily clinic visits but may be better for severe, long-term heroin addiction. Vivitrol (monthly injection) is another option for those who prefer a non-opioid approach. Your Trailhead treatment team will recommend the best fit for your situation.
Naloxone (Narcan) reverses opioid overdoses and can save a life in minutes. Yes — if a family member uses heroin, you should carry naloxone. It’s available at NH pharmacies without a prescription. We provide naloxone training as part of our family education program.
PHP typically lasts 20–30 days, IOP runs 60–90 days, and MAT maintenance may continue for months or years. Research shows longer MAT duration correlates with better outcomes. Most clients benefit from a step-down approach with ongoing MAT support.
Yes. Most major insurance plans cover heroin and opioid addiction treatment, including MAT. We’re in-network with Anthem BCBS, Point 32 Health, Tricare, Uprise Health, and WellSense. Our team verifies benefits in minutes.
Absolutely. We offer individual family therapy sessions plus monthly Zoom education groups. For heroin addiction specifically, we include naloxone training and education on opioid-specific risks so families can be prepared. Open to any family member 18+.
Yes. Heroin addiction commonly co-occurs with depression, anxiety, PTSD, and other mental health conditions. Our clinicians are trained in dual-diagnosis treatment, addressing both the addiction and underlying mental health issues simultaneously through integrated therapy.
While we do not operate a needle exchange, we support harm reduction principles and can connect clients with local syringe service programs. Our focus is on comprehensive treatment including MAT, therapy, and relapse prevention — meeting people where they are on their recovery journey.
Comprehensive MAT options (Suboxone, Methadone, Vivitrol) managed by our on-site NP, family-style environment, staff with lived opioid recovery experience, 12:1 counselor caseloads, flexible scheduling, naloxone training for families, and same-day admission when clinically appropriate.
