How to Help a Loved One with OCD

Educate Yourself First

Understand that OCD is a neurobiological condition, not a personality quirk or choice. Learn how the OCD cycle works — the obsessions trigger anxiety, and compulsions provide temporary relief but reinforce the pattern. Knowledge helps you respond with compassion.

Stop Accommodating OCD

Family accommodation — participating in rituals, providing reassurance, or adjusting routines around OCD — is natural but ultimately reinforces the disorder. Work with a therapist to gradually reduce accommodations while maintaining a supportive relationship.

Resist Providing Reassurance

When your loved one asks “Are you sure it’s safe?” or “Did I lock the door?”, reassurance feels helpful but feeds the OCD cycle. Instead, acknowledge their distress: “I know this feels scary, and I trust you can handle the uncertainty.”

Have the Conversation

What If They Refuse Treatment?

It’s common for people struggling with OCD to resist treatment, often because ERP involves facing fears directly. This doesn’t mean progress is impossible — it means a different approach may be needed.

A structured conversation with a mental health professional can help your loved one understand that OCD is treatable and that ERP, while challenging, is highly effective. Therapists can address fears about treatment and explain the process in a compassionate, non-pressuring way.
Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change — helping them arrive at the decision to seek help on their own terms.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept, and maintain those limits consistently.
