Recognize the Signs

Signs & Symptoms of Co-Occurring Disorders

Co-occurring disorders involve both a substance use disorder and a mental health condition interacting and reinforcing each other. Select a category to learn more.

Substance Use Warning Signs

  • Using substances to cope with anxiety, depression, or emotional pain
  • Needing more of a substance to achieve the same relief from mental health symptoms
  • Failed attempts to stop using despite worsening psychiatric symptoms
  • Withdrawal symptoms that mimic or intensify underlying mental health conditions

Mental Health Warning Signs

  • Persistent depression, anxiety, or mood swings that worsen with substance use
  • Difficulty functioning at work, school, or in relationships beyond what substance use alone explains
  • Trauma responses, flashbacks, or hypervigilance alongside addictive behaviors
  • Feelings of hopelessness or suicidal thoughts, especially during withdrawal periods

How Both Conditions Interact

  • Mental health symptoms trigger substance use, and substance use worsens mental health — a reinforcing cycle
  • Previous treatment for one condition failed because the other was left unaddressed
  • Medications for mental health are less effective due to active substance use
  • Repeated relapses driven by untreated psychiatric symptoms like PTSD, bipolar disorder, or severe anxiety

If you or a loved one is experiencing signs of both addiction and mental health challenges, integrated treatment can help.

The Reality of Dual Diagnosis

Co-Occurring Disorders in America

According to SAMHSA, approximately 9.2 million American adults experience co-occurring mental health and substance use disorders. Despite this staggering number, only 7.4% receive treatment for both conditions simultaneously.

The treatment gap is devastating: 52.5% receive no treatment at all, while many others receive care for only one condition — leaving them at high risk for relapse and worsening symptoms. In New Hampshire, where substance use rates exceed national averages, the need for integrated dual-diagnosis treatment is especially urgent.

These aren’t just statistics — they represent families in Salem, Derry, Nashua, Manchester, and across the region who need access to truly integrated treatment that addresses both conditions together.

How to Help a Loved One with Co-Occurring Disorders

Understand Both Conditions

Co-occurring disorders mean your loved one is dealing with two medical conditions that interact and reinforce each other. Their substance use may be an attempt to manage untreated mental health symptoms, and their mental health may worsen because of substance use. Understanding this cycle is the first step.

Recognize Why Previous Treatment May Have Failed

If your loved one has been to treatment before and relapsed, it may be because only one condition was addressed. Treating addiction without treating the underlying depression, anxiety, or PTSD often leads to relapse. Integrated treatment changes the equation.

Set Boundaries with Compassion

Communicate clear limits: “I love you, and I want you to get help for both what you’re going through emotionally and the substance use.” Follow through consistently. Boundaries protect your wellbeing and can motivate change.

Have the Conversation

Choose a calm moment. Use “I” statements: “I can see you’re struggling with more than one thing” rather than labeling or diagnosing. Offer specific help — like calling Trailhead together for a dual-diagnosis assessment.

Take Care of Yourself

Living with someone who has co-occurring disorders is especially challenging. Consider therapy for yourself, join a support group like NAMI or Al-Anon, and maintain your own routines. Your wellbeing matters too.

Family Support at Trailhead

Our family support program is open to all family members 18+, past and present clients. We offer individual family therapy sessions and monthly Zoom education groups covering how addiction and mental health conditions interact within family systems.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with co-occurring disorders to resist treatment. They may not recognize the connection between their substance use and mental health, or fear stigma around both. This doesn’t mean recovery is impossible.

A structured, guided intervention led by a professional can break through denial and help your loved one see the impact of both their substance use and untreated mental health symptoms on themselves and their family. Interventionists trained in dual-diagnosis understand the complexity involved.
Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change — helping them arrive at the decision to seek help on their own terms.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept, and maintain those limits consistently.
People move through precontemplation, contemplation, preparation, action, and maintenance. Meeting them where they are is more effective than pushing. Recognizing which stage they’re in helps you respond appropriately.
If there’s immediate danger — suicidal thoughts, severe withdrawal symptoms, or risk of harm — call 911 or the 988 Suicide & Crisis Lifeline. Our admissions team can also guide you through next steps and discuss intervention options.

Frequently Asked Questions

Co-Occurring Disorders Treatment FAQ

Co-occurring disorders (also called dual diagnosis) means having both a substance use disorder and a mental health condition at the same time. Common combinations include addiction with depression, anxiety, PTSD, bipolar disorder, or ADHD. The conditions interact and often make each other worse.
Neither — both conditions are treated simultaneously. Research consistently shows that integrated treatment addressing addiction and mental health together produces better outcomes than treating them sequentially. At Trailhead, your treatment plan addresses both from day one.
Yes. Our clinicians hold credentials in both substance use treatment (LADC) and mental health counseling (LCMHC, LICSW). They understand how both conditions interact and can provide truly integrated therapy rather than treating each in isolation.
Our on-site Nurse Practitioner manages both MAT for addiction (Vivitrol, Naltrexone, Suboxone) and psychiatric medications (antidepressants, mood stabilizers, anti-anxiety) as one coordinated plan, ensuring no interactions and optimal effectiveness for both conditions.
Absolutely. Our Evening Professional Track (6–9 PM, Mon-Thu) provides full dual-diagnosis treatment during evening hours. We also offer afternoon IOP and telehealth options with flexible daily scheduling.
Most major plans cover integrated co-occurring disorders treatment. We’re in-network with Anthem BCBS, Point 32 Health, Tricare, Uprise Health, and WellSense. Our team verifies benefits for both substance use and mental health coverage in minutes.
Many of our clients have been through treatment programs that only addressed their addiction. If the underlying mental health condition was left untreated, relapse is common. Integrated treatment that addresses both conditions simultaneously often produces different — and better — outcomes.
We treat depression, anxiety disorders, PTSD and complex trauma, bipolar disorder, ADHD, and other mood and personality disorders alongside substance use disorders. Our clinicians are trained to address the interaction between these conditions.
Yes. We offer individual family therapy sessions plus monthly Zoom education groups covering how addiction and mental health conditions interact, codependency, and family systems. Open to any family member 18+.
Truly integrated treatment where both conditions are treated by the same clinical team simultaneously — not in separate groups or separate programs. Combined with dual-trained clinicians, coordinated medication management, flexible scheduling, and same-day admission.
