Self-harm — also called non-suicidal self-injury (NSSI) — is a way of coping with overwhelming emotions. Recognizing the signs is the first step toward getting help.
Warning Signs Early indicators may include unexplained cuts, scratches, bruises, or burns, often on the arms, thighs, or torso. You might notice someone wearing long sleeves or pants even in warm weather, withdrawing from friends and family, or expressing feelings of worthlessness, hopelessness, or emotional numbness.
Escalating As self-harm becomes a more established coping pattern, the behaviors may intensify. Cutting, burning, hitting, or scratching may become more frequent or severe. Keeping sharp objects or first aid supplies hidden is common. Isolation increases, school or work performance declines, and the person may express difficulty controlling the urge to harm themselves.
Crisis Level At this stage, self-harm may co-occur with suicidal thoughts or attempts. Injuries may require medical attention. The person may feel trapped in the cycle — using self-harm to manage emotional pain but experiencing increasing shame, guilt, and emotional distress as a result. Professional intervention is critical.
