What Sets Our Outpatient Program Apart

Explore Your Treatment Experience

Schedule

Once per week, with session times arranged directly with your clinician. Flexible scheduling means you can maintain your work, family, and personal commitments while continuing your recovery.

Duration

Ongoing, as clinically appropriate. There is no fixed end date — your treatment length is determined by your progress, goals, and clinical needs, ensuring you receive support for as long as you need it.

What's Included

Weekly individual therapy, one group session per week, continued case management, and coordination of external referrals. Everything you need to sustain your recovery at a lighter clinical level.

Who It's For

Ideal for those stepping down from IOP, individuals seeking ongoing maintenance support, or anyone needing minimal clinical oversight while living independently in their recovery journey.

Medication Management

Continue working with our clinical team on medication management, including MAT options like Suboxone, Vivitrol, and Naltrexone — ensuring continuity of care from higher levels of treatment.

External Referral Coordination

Your care team coordinates with outside providers, community resources, and support groups to build a comprehensive recovery network that extends well beyond our walls.

Individual Session (1 Hour)

Your weekly one-on-one session with a licensed therapist is the cornerstone of outpatient care. Together, you'll review your progress, address emerging challenges, reinforce coping strategies learned in IOP or PHP, and set goals for the week ahead. These sessions are tailored to your evolving needs as you build independence in recovery.

Group Session (1.5 Hours)

Once per week, you'll join a small peer group for collaborative support. These sessions focus on maintaining the skills you've built — relapse prevention, emotional regulation, interpersonal effectiveness, and real-world problem-solving. The group dynamic provides accountability and connection with others at a similar stage of recovery.

Independent Recovery Practice

Between sessions, you'll apply what you've learned to daily life. Your therapist provides structured exercises, journaling prompts, and self-monitoring tools to keep you engaged with your recovery throughout the week. This is where the real work happens — practicing skills in the environment where they matter most.

Continued Community Connection

Outpatient clients remain connected to the Trailhead community through alumni events, family support groups, and access to recovery resources. You're never truly on your own — your support network continues to grow even as your clinical needs decrease.

1

Career & Life Integration

With once-per-week scheduling, you're fully engaged in your career, education, and daily life. Your case manager continues to support practical needs — job development, financial planning, and community resources.

2

Family Support Access

Family members remain welcome in the recovery process. Monthly Zoom family support groups (open to any family member 18+) and coordinated family sessions continue through outpatient care.

3

External Support Network

Your therapist helps you build and strengthen connections outside of treatment — community recovery groups, peer support networks, wellness resources, and external providers who become part of your long-term plan.

4

Alumni Program Transition

When you and your team agree you're ready, you'll transition into our alumni program — maintaining access to community events, support groups, and the Trailhead network that supported your recovery from the beginning.

5

Relapse Prevention Planning

Before graduating, you'll develop a comprehensive relapse prevention plan with your therapist — identifying triggers, reinforcing coping strategies, and establishing a clear action plan for maintaining long-term sobriety.

6

Continued Access to Care

Recovery doesn't end at graduation. Former clients can return for booster sessions, re-engage with group support, or step back up to a higher level of care if needed — the door is always open.

Your Recovery Journey

Treatment Phases

Phase 1: Transition Planning Phase 2: Supported Independence Phase 3: Community Integration Phase 4: Graduation & Alumni

Transition Planning

Your outpatient journey begins with a smooth transition from IOP. Your clinical team assesses your readiness, reviews your progress, and collaboratively sets new goals tailored to this lighter level of care. Together, you'll establish a weekly rhythm that supports continued growth while honoring your independence.

Supported Independence

Weekly check-ins with your therapist and group sessions provide a consistent touchpoint as you navigate daily life in recovery. This phase focuses on skill reinforcement — applying what you learned in PHP and IOP to real-world challenges, with professional guidance available when you need it.

Community Integration

As your confidence grows, the focus shifts to building a robust external support network. Your care team helps you connect with recovery community groups, peer support organizations, wellness resources, and other providers who become part of your long-term recovery ecosystem.

Graduation & Alumni

When you and your clinical team agree you're ready, you'll transition into our alumni program. This isn't an ending — it's a new chapter. You'll maintain access to community events, support groups, booster sessions, and the Trailhead network. If challenges arise, you can always step back up to a higher level of care.

Common Questions

Answers About Outpatient

What is an Outpatient Program (OP)?
An Outpatient Program is the lightest level of clinical care we offer. It provides once-per-week sessions — including individual therapy and group support — designed for people who have completed IOP or PHP and are ready to maintain their recovery with minimal clinical oversight while living independently.
What's the difference between IOP and OP?
IOP (Intensive Outpatient Program) involves 3-4 sessions per week at 3.5 hours each. OP (Outpatient Program) is a step down from IOP — meeting just once per week for individual therapy and a group session. OP is designed for ongoing maintenance and support, while IOP provides more intensive structure and skill building.
How often do I attend outpatient sessions?
Outpatient sessions are scheduled once per week. Your session time is arranged directly with your clinician, giving you maximum flexibility to fit treatment around work, family, and other commitments. Each week includes one individual therapy session (1 hour) and one group session (1.5 hours).
How long does outpatient treatment last?
Outpatient treatment is ongoing, with duration based on your individual needs and clinical progress. There's no fixed end date — you and your therapist will collaboratively determine when you're ready to transition to the alumni program. Some clients attend for a few months, others for longer.
Can I work full-time while in OP?
Absolutely. Outpatient is specifically designed for people maintaining full-time work, school, or family responsibilities. With just one session per week scheduled at a time that works for you, OP fits seamlessly into a busy life while ensuring you still have professional clinical support.
What happens during an outpatient session?
Your weekly individual session focuses on reviewing your progress, addressing emerging challenges, reinforcing coping strategies, and setting goals for the week ahead. Group sessions provide peer support, accountability, and collaborative problem-solving with others at a similar stage of recovery.
Is outpatient treatment effective for addiction?
Yes — research consistently shows that continued outpatient support after more intensive treatment significantly reduces relapse rates and improves long-term outcomes. OP provides the ongoing clinical touchpoint that helps you maintain the skills and habits built during PHP and IOP, while practicing recovery in real-world settings.
Do I need to complete IOP before starting OP?
Not necessarily. While most clients transition to OP after completing IOP, some individuals may be clinically appropriate for outpatient care based on their assessment. Your clinical team will work with you to determine the right level of care for your current needs and recovery stage.
Does insurance cover outpatient treatment?
Most insurance plans cover outpatient treatment. We're in-network with Anthem BCBS, Point 32 (Harvard Pilgrim + Tufts), Tricare, Uprise Health, and WellSense NH Medicaid. Our admissions team handles the verification process, and we also offer affordable self-pay options. Call (857) 312-1697 for a free insurance check.
What support is available after outpatient ends?
When you graduate from OP, you transition into our alumni program — which provides continued access to community events, support groups, and recovery resources. If challenges arise, you can always return for booster sessions or step back up to a higher level of care. The Trailhead community is a lifelong resource.
