Recognize the Signs

Signs & Symptoms of OCD

Obsessive-Compulsive Disorder (OCD) involves a cycle of intrusive thoughts (obsessions) and repetitive behaviors (compulsions). Recognizing these patterns is the first step toward breaking free.

Obsessions OCD obsessions are unwanted, intrusive thoughts, images, or urges that cause significant anxiety. Common themes include contamination fears, harm to self or others, symmetry and order, religious or moral scrupulosity, and disturbing thoughts that feel completely at odds with who you are. These thoughts are not choices — they intrude despite your best efforts to push them away.

Compulsions To manage the distress from obsessions, people with OCD develop compulsive rituals. Excessive hand-washing, checking, counting, arranging, or mental rituals like repeating phrases become time-consuming and exhausting. You may recognize these behaviors are irrational, but the anxiety feels unbearable without performing them. Compulsions provide temporary relief but ultimately reinforce the OCD cycle.

Life Impact Severe OCD can consume hours of every day. Work, school, and relationships suffer as rituals expand and avoidance increases. Some people become housebound. The shame and secrecy surrounding intrusive thoughts often prevents people from seeking help. Without treatment, OCD rarely improves on its own and tends to worsen over time. But with evidence-based treatment like ERP, significant improvement is possible.

If you or a loved one is experiencing these obsessions and compulsions, effective treatment is available. OCD is one of the most treatable mental health conditions when the right approach is used.

For Family Members

How to Help a Loved One with OCD

Supporting someone through OCD is one of the hardest things a family can do. These evidence-based steps can help you take care of both yourself and your loved one.

Educate Yourself About OCD

OCD is a neurobiological condition — not a personality quirk or choice. Understanding that intrusive thoughts are involuntary and that compulsions provide only temporary relief helps you respond with compassion rather than frustration.

Set Clear, Loving Boundaries

Communicate limits with compassion: “I love you, and I want to support your recovery, not your OCD.” Gradually reduce accommodation of rituals with guidance from their therapist.

Stop Accommodating OCD Rituals

Family accommodation — participating in rituals, providing reassurance, or modifying routines to reduce their anxiety — is one of the biggest barriers to OCD recovery. Work with their therapist to gradually reduce accommodation in a supportive way.

Consider Family Therapy

Trailhead offers individual family therapy sessions and monthly Zoom education groups covering OCD education, reducing family accommodation, and supporting ERP practice at home. Open to family members 18+.

Take Care of Your Own Mental Health

Your wellbeing matters. Consider therapy for yourself, connect with the International OCD Foundation or NAMI for family resources, and maintain your own routines and social connections.

Avoid Providing Reassurance

When they ask “Are you sure it’s safe?” or “Did I do that right?” — these are reassurance-seeking compulsions. Instead of answering, gently redirect: “That sounds like OCD talking. What would your therapist suggest?”

Our family support program is open to all family members 18+, for current and past clients.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with OCD to resist treatment — often because they fear exposures or feel shame about their thoughts. Here’s how to respond effectively.

Do This

  • Express concern with empathy and “I” statements
  • Set clear boundaries and follow through consistently
  • Research ERP-based treatment options so you’re prepared when they’re ready
  • Share information about ERP success rates and what treatment actually looks like
  • Take care of your own mental health and wellbeing
  • Connect with the International OCD Foundation for family resources
  • Call our admissions team for guidance — even if they’re not ready

Don’t Do This

  • Issue ultimatums you won’t follow through on
  • Participate in their rituals or provide reassurance to reduce their anxiety
  • Say “just stop doing it” or minimize their experience
  • Use guilt, shame, or blame as motivation
  • Ignore worsening symptoms hoping OCD will resolve on its own
  • Sacrifice your own health and stability accommodating OCD rituals
  • Give up — people enter recovery at all stages of readiness

If there’s immediate danger — suicidal thoughts, severe depression, or risk of harm — call 911 or the 988 Suicide & Crisis Lifeline.

Frequently Asked Questions

OCD Treatment FAQ

Common signs include persistent unwanted thoughts (obsessions) about contamination, harm, symmetry, or taboo subjects, followed by repetitive behaviors (compulsions) like washing, checking, counting, or mental rituals. If these thoughts and behaviors consume more than an hour daily or significantly interfere with your life, it may be OCD.
Treatment length varies by OCD severity. Our PHP program typically lasts 20–30 days of intensive ERP, IOP runs 60–90 days, and ongoing outpatient support continues as needed. Many clients see significant improvement within 12–20 ERP sessions, though some benefit from longer-term support.
Exposure and Response Prevention (ERP) is the gold standard treatment for OCD. It involves gradually facing feared situations (exposures) while resisting the urge to perform compulsions (response prevention). Over time, your brain learns that the feared outcomes don’t happen and anxiety naturally decreases without rituals.
Yes. Our on-site Nurse Practitioner can prescribe SSRIs at OCD-therapeutic doses (often higher than doses used for depression) and SRI medications. For treatment-resistant OCD, augmentation strategies may be explored. Medication is most effective when combined with ERP therapy.
Absolutely. Our Evening Professional Track (6–9 PM, Monday through Thursday) is specifically designed for working adults managing OCD. We also offer afternoon IOP sessions and telehealth options. In fact, practicing ERP in your daily environment can enhance treatment effectiveness.
Most major insurance plans cover OCD treatment under mental health parity laws. We are in-network with Anthem BCBS, Point 32 Health (Harvard Pilgrim + Tufts), Tricare, Uprise Health, and WellSense (NH Medicaid). Our admissions team can verify your benefits in minutes.
Our primary approach is Exposure and Response Prevention (ERP), the most effective therapy for OCD. We also incorporate Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT) for building psychological flexibility, and holistic approaches like mindfulness meditation and breathwork that complement ERP practice.
Yes — family involvement is crucial for OCD treatment. We offer individual family therapy sessions to address accommodation patterns, plus monthly family education groups via Zoom covering OCD education, reducing accommodation, and supporting ERP at home. Open to any family member 18+.
We plan for aftercare from day one. Options include stepping down to IOP or OP, enrolling in our alumni program, referrals to external therapists and psychiatrists, continued access to family support groups, and supportive living coordination.
Our differentiators include a family-style environment where staff know every client by name, flexible scheduling across morning/afternoon/evening tracks, staff with both professional credentials and lived recovery experience, 12:1 counselor caseloads, and same-day admission availability when clinically appropriate.
