Obsessive-Compulsive Disorder (OCD) involves a cycle of intrusive thoughts (obsessions) and repetitive behaviors (compulsions). Recognizing these patterns is the first step toward breaking free.
Obsessions OCD obsessions are unwanted, intrusive thoughts, images, or urges that cause significant anxiety. Common themes include contamination fears, harm to self or others, symmetry and order, religious or moral scrupulosity, and disturbing thoughts that feel completely at odds with who you are. These thoughts are not choices — they intrude despite your best efforts to push them away.
Compulsions To manage the distress from obsessions, people with OCD develop compulsive rituals. Excessive hand-washing, checking, counting, arranging, or mental rituals like repeating phrases become time-consuming and exhausting. You may recognize these behaviors are irrational, but the anxiety feels unbearable without performing them. Compulsions provide temporary relief but ultimately reinforce the OCD cycle.
Life Impact Severe OCD can consume hours of every day. Work, school, and relationships suffer as rituals expand and avoidance increases. Some people become housebound. The shame and secrecy surrounding intrusive thoughts often prevents people from seeking help. Without treatment, OCD rarely improves on its own and tends to worsen over time. But with evidence-based treatment like ERP, significant improvement is possible.
If you or a loved one is experiencing these obsessions and compulsions, effective treatment is available. OCD is one of the most treatable mental health conditions when the right approach is used.

Supporting someone through OCD is one of the hardest things a family can do. These evidence-based steps can help you take care of both yourself and your loved one.
OCD is a neurobiological condition — not a personality quirk or choice. Understanding that intrusive thoughts are involuntary and that compulsions provide only temporary relief helps you respond with compassion rather than frustration.
Communicate limits with compassion: “I love you, and I want to support your recovery, not your OCD.” Gradually reduce accommodation of rituals with guidance from their therapist.
Family accommodation — participating in rituals, providing reassurance, or modifying routines to reduce their anxiety — is one of the biggest barriers to OCD recovery. Work with their therapist to gradually reduce accommodation in a supportive way.
Trailhead offers individual family therapy sessions and monthly Zoom education groups covering OCD education, reducing family accommodation, and supporting ERP practice at home. Open to family members 18+.
Your wellbeing matters. Consider therapy for yourself, connect with the International OCD Foundation or NAMI for family resources, and maintain your own routines and social connections.
When they ask “Are you sure it’s safe?” or “Did I do that right?” — these are reassurance-seeking compulsions. Instead of answering, gently redirect: “That sounds like OCD talking. What would your therapist suggest?”
Our family support program is open to all family members 18+, for current and past clients.
