Discovering that someone you love is hurting themselves is frightening. These approaches can help you respond effectively.
Our family support program is open to all family members 18+, past and present clients.
Call for Family SupportIt’s common for people struggling with self-harm to resist treatment. These approaches can help.
Choose a calm, private moment. Use “I” statements: “I’ve noticed some marks on your arms and I’m worried about you. I care about you and I want to help.” Avoid confrontation, ultimatums, or demands to stop immediately — shame drives self-harm deeper into secrecy.
Self-harm carries enormous shame. Normalize that many people struggle with it and that effective treatment exists. Share that DBT has helped thousands of people develop healthier coping skills. Frame treatment as gaining new tools, not “fixing” them.
Give them options: “Would you prefer to talk to a therapist, call a crisis line, or let me help you find a program?” Offering choices respects their autonomy. For Trailhead, telehealth is available if in-person feels too overwhelming at first.
Even if they refuse treatment today, keep the door open. Check in regularly without pressuring. Let them know you care and that help is available whenever they’re ready. Isolation worsens self-harm — your consistent presence matters more than you know.
If there is immediate danger — suicidal statements, deep wounds requiring medical attention, or escalating self-harm severity — call 911 immediately. You can also call the 988 Suicide & Crisis Lifeline (call or text 988) or text HOME to 741741 (Crisis Text Line). Do not leave them alone.
Our admissions team can guide you even if your loved one isn’t ready.
