Recognize the Signs

Signs & Symptoms of Self-Harm

Self-harm is a coping mechanism, not a choice. Understanding the warning signs can help you or your loved one get the right support.

Warning Signs

Unexplained cuts, bruises, burns, or scratches — often on arms, thighs, or stomach
Wearing long sleeves or pants in warm weather to conceal marks
Keeping sharp objects, lighters, or other items that could be used for self-injury
Social withdrawal and increasing secrecy about time spent alone

Behavioral Patterns

Cutting, burning, hitting, or scratching as a way to cope with emotional pain
Feeling a brief sense of relief or release after self-injury, followed by shame or guilt
Difficulty identifying or expressing emotions (alexithymia)
Statements of feeling numb, empty, or needing to “feel something”

Escalating Risk

Increasing frequency or severity of self-harm behaviors
Self-harm requiring medical attention (deep cuts, infections, significant burns)
Expressing hopelessness, worthlessness, or statements about wanting to die
Self-harm combined with suicidal ideation — this requires immediate crisis intervention

In crisis? Call the 988 Suicide & Crisis Lifeline (call or text 988) or text HOME to 741741 (Crisis Text Line).

For Family Members

How to Help a Loved One Who Self-Harms

Discovering that someone you love is hurting themselves is frightening. These approaches can help you respond effectively.

📚

Understand the Why

Self-harm is a coping mechanism for overwhelming emotional pain — not attention-seeking. Understanding this helps you respond with compassion rather than panic.

🛑

Don’t React with Anger

Anger, disgust, or ultimatums drive self-harm underground. Stay calm and express concern without judgment. Your reaction shapes whether they’ll open up again.

🤝

Listen Without Fixing

Sometimes the most powerful thing you can do is listen. Say “I’m here for you” and mean it. Don’t try to solve the problem in the moment.

👥

Family Therapy

Family sessions help everyone understand self-harm, improve communication, and learn how to create a supportive home environment for healing.

💚

Self-Care First

Caring for someone who self-harms is emotionally exhausting. Your own mental health matters. Seek your own support — you can’t pour from an empty cup.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with self-harm to resist treatment. These approaches can help.

Express Concern Without Judgment

Choose a calm, private moment. Use “I” statements: “I’ve noticed some marks on your arms and I’m worried about you. I care about you and I want to help.” Avoid confrontation, ultimatums, or demands to stop immediately — shame drives self-harm deeper into secrecy.

Reduce Shame and Stigma

Self-harm carries enormous shame. Normalize that many people struggle with it and that effective treatment exists. Share that DBT has helped thousands of people develop healthier coping skills. Frame treatment as gaining new tools, not “fixing” them.

Offer Choices, Not Ultimatums

Give them options: “Would you prefer to talk to a therapist, call a crisis line, or let me help you find a program?” Offering choices respects their autonomy. For Trailhead, telehealth is available if in-person feels too overwhelming at first.

Stay Connected

Even if they refuse treatment today, keep the door open. Check in regularly without pressuring. Let them know you care and that help is available whenever they’re ready. Isolation worsens self-harm — your consistent presence matters more than you know.

Know When to Call for Help

If there is immediate danger — suicidal statements, deep wounds requiring medical attention, or escalating self-harm severity — call 911 immediately. You can also call the 988 Suicide & Crisis Lifeline (call or text 988) or text HOME to 741741 (Crisis Text Line). Do not leave them alone.

Our admissions team can guide you even if your loved one isn’t ready.

Frequently Asked Questions

Self-Harm Treatment FAQ

NSSI refers to the deliberate, self-inflicted destruction of body tissue without suicidal intent. It includes cutting, burning, hitting, scratching, or other forms of self-injury used to cope with overwhelming emotions. It is a serious mental health concern that responds well to evidence-based treatment.
Not necessarily. Most self-harm is a coping mechanism for emotional pain, not a suicide attempt. However, self-harm does increase the risk of suicide over time, which is why treatment is so important. Any mention of wanting to die should be taken seriously — call 988 or text HOME to 741741 if there is immediate concern.
Self-harm serves different functions for different people: releasing overwhelming emotional pain, feeling something when emotionally numb, expressing feelings that can’t be put into words, punishing oneself, or regaining a sense of control. Understanding the “why” is a key part of treatment at Trailhead.
Treatment length varies by individual need and severity. PHP typically lasts 20–30 days, IOP runs 60–90 days, and ongoing outpatient support continues as needed. DBT skills take time to internalize — most clients benefit from at least 12 weeks of structured treatment.
Dialectical Behavior Therapy (DBT) was originally developed specifically for self-harm and suicidal behaviors. It teaches four core skill sets: mindfulness, distress tolerance, emotional regulation, and interpersonal effectiveness. Research shows it significantly reduces self-harm frequency and severity.
Medication can help address underlying conditions that contribute to self-harm, such as depression, anxiety, or emotional dysregulation. Our on-site NP evaluates each client individually. Medication always complements DBT therapy, never replaces it.
Yes — family involvement is encouraged. We offer individual family therapy sessions and monthly Zoom family education groups. Families learn about self-harm, effective communication, and how to support treatment without enabling. Open to any family member 18+.
Most major insurance plans cover mental health treatment including self-harm. We’re in-network with Anthem BCBS, Point 32 Health, Tricare, Uprise Health, and WellSense NH Medicaid. Our admissions team can verify your benefits in minutes.
Absolutely. Our Evening Professional Track (6–9 PM, Mon–Thu) is designed for working adults. We also offer afternoon IOP sessions and telehealth. You can switch between time slots daily based on your schedule.
We plan for aftercare from day one. Options include stepping down to IOP or OP, enrolling in our alumni program, referrals to external therapists, continued family support groups, and sober living coordination.
