Use Motivational Approaches

Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change. Ask open-ended questions: “What would your life look like without heroin?” Let them arrive at their own motivation.

Set Clear Boundaries

Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific: “If I find heroin or needles in the house, I will stay at my sister’s.” Then follow through consistently — empty threats enable continued use.

Know When to Call for Help

Heroin Addiction Treatment FAQ

Common signs include track marks or needle scars, pinpoint pupils, nodding off at odd times, severe weight loss, secretive behavior, financial desperation, possession of paraphernalia (needles, burnt spoons, rubber tubing), and painful withdrawal symptoms when not using.
This is an extremely common pathway. When prescription opioids become too expensive, hard to obtain, or the prescription runs out, many people turn to heroin because it’s cheaper and more accessible. About 80% of heroin users started with prescription painkillers. The progression can happen in weeks or months.
Trailhead does not provide medical detox on-site. However, we coordinate closely with trusted detox facilities and can begin Suboxone stabilization immediately upon admission to our PHP program, typically within 24–48 hours of completing detox. Our admissions team will arrange the transition.
Treatment length varies by individual need. PHP typically lasts 20–30 days, IOP runs 60–90 days, and ongoing outpatient support continues as needed. Many heroin clients benefit from long-term MAT (often 12+ months) combined with therapy to prevent relapse.
Our on-site Nurse Practitioner can prescribe Suboxone (buprenorphine/naloxone) and Vivitrol (naltrexone). Methadone is available through coordinated referral to a licensed OTP. These medications eliminate withdrawal, reduce cravings, and block opioid receptors. MAT is always combined with therapy for a comprehensive approach.
