Voices From the Trail

What Outpatient Looks Like

Your Weekly Recovery Rhythm

Schedule

Once a Week, Every Week

Outpatient at Trailhead meets once per week — a flexible, ongoing commitment designed to fit around your work, school, and family responsibilities. Sessions are scheduled at a time that works for you, with morning, afternoon, and evening availability.

This level of care is designed for clients stepping down from IOP who have built a strong foundation and are ready for greater independence while maintaining clinical support and accountability.

Included Services

Weekly Individual & Group Therapy

Each week includes individual therapy with your licensed clinician and a group session that reinforces recovery skills. Individual sessions use evidence-based modalities — CBT, DBT, ACT — tailored to your evolving needs as you build independence.

Group sessions maintain the community connection that's vital to long-term sobriety. You'll continue refining relapse prevention strategies, practicing emotional regulation, and sharing progress with peers who understand the journey.

Case Management

Practical Support for Real Life

Your case manager remains your advocate during outpatient — coordinating employment resources, housing assistance, legal paperwork, and community referrals. As you regain independence, case management shifts focus toward long-term stability and self-sufficiency.

External referrals connect you with therapists, psychiatrists, support groups, and community resources beyond Trailhead. The goal is a self-sustaining recovery network that carries you forward long after graduation.

Who It's For

Stepping Down from IOP

Outpatient is the next step for clients who have completed IOP and demonstrated sustained progress. It's ideal for adults who are stable in their recovery, engaged in work or education, and ready to practice the skills they've built with less clinical oversight.

Medication management continues through outpatient — your Nurse Practitioner monitors Suboxone, Vivitrol, or other medications and adjusts as your recovery evolves. Family therapy remains available to strengthen the relationships that support your long-term success.
