Schedule & Tracks

Three Tracks for Your Life

IOP at Trailhead offers three flexible tracks: Morning (9:00 AM – 12:30 PM), Afternoon (12:30 – 3:30 PM), and Evening (6:00 – 9:00 PM), Monday through Thursday. Choose the track that fits your work, family, or school schedule — and switch between tracks as your life changes.

The program runs 60 to 90 days, with duration individualized based on your progress and clinical recommendation. IOP is our primary step-down from PHP, giving you continued intensive support while you re-engage with daily life.

In the Group Room

Evidence-Based Group Therapy

Group therapy is the core of IOP at Trailhead. Each session includes focused skills groups in CBT, DBT, and ACT — building emotional regulation, distress tolerance, and psychological flexibility. Psychoeducation groups deepen your understanding of addiction and co-occurring mental health conditions.

Relapse prevention sessions equip you with concrete strategies for real-world situations. Your treatment plan adapts to your preferences — 12-Step, SMART Recovery, Dharma Recovery, or a personalized blend. Snacks are provided during every IOP session.

Clinical Care Within 72 Hours

Within 72 hours of admission, you'll meet with our on-site Nurse Practitioner for a comprehensive medical evaluation and personalized medication management plan — including Suboxone, Vivitrol, or other FDA-approved medications when clinically appropriate.

Weekly individual therapy pairs you with a licensed clinician who works one-on-one using Cognitive Behavioral Therapy to reshape the thought patterns that fuel addiction. Collaborative treatment planning ensures every clinical decision aligns with your goals. The 12:1 counselor-to-client ratio means you get the attention you deserve.

FAQ

Common Questions

Most clients complete IOP in 60 to 90 days, though duration is individualized based on progress and clinical recommendation. Your treatment team adjusts the timeline to your needs — not the other way around.

Absolutely — that's one of IOP's biggest advantages. Our evening track (6:00–9:00 PM, Mon–Thu) is designed specifically for working professionals. Morning and afternoon tracks also allow flexibility around work, school, or family obligations.

We offer three tracks: Morning (9:00 AM – 12:30 PM), Afternoon (12:30 – 3:30 PM), and Evening (6:00 – 9:00 PM), all Monday through Thursday. You can switch between tracks as your schedule changes.

Most clients have some or all of IOP covered by insurance. We accept Anthem BCBS, Point 32 (Harvard Pilgrim + Tufts), Tricare, Uprise Health, and WellSense NH Medicaid. We verify your coverage before admission — call (857) 312-1697 or use our online form.

PHP is our most intensive level — full days (9 AM–3:30 PM) with lunch provided. IOP is a step-down: 3 to 3.5-hour sessions with morning, afternoon, or evening options, running 60–90 days. Both include individual therapy, group work, and medication management.

Yes. Telehealth and hybrid options are available for all program levels including IOP. You can attend sessions virtually or mix in-person and virtual based on your needs and preferences.

IOP is ideal for clients stepping down from PHP who need continued intensive support, working professionals who cannot attend full-day programming, and anyone whose clinical assessment indicates a need for structured outpatient care while maintaining daily responsibilities.

Absolutely. Individual family therapy sessions are available, and we host monthly family support groups via Zoom — open to any family member 18+, covering codependency, family systems, and how to support recovery effectively.

Your treatment team coordinates a step-down to outpatient care, connects you with external therapists and psychiatrists, and enrolls you in our alumni program for ongoing community support and lifelong connection.

Three flexible tracks including an evening option, NP access within 72 hours, 12:1 counselor caseloads, CBT/DBT/ACT evidence-based modalities, weekly individual therapy, holistic programming, dual NH and MA licensing, family support, and same-day admissions. We adapt to your life instead of demanding you adapt to ours.
