Your Program

Four Chapters of Healing

Our PHP isn't a one-size-fits-all curriculum. It's a recovery journey organized around the outcomes that matter most — clarity, resilience, connection, and independence.

Chapter I

Finding Clarity

Recovery begins with seeing clearly — understanding what brought you here and what tools can carry you forward. Your day runs 9 AM to 3:30 PM, Monday through Friday, and within 24 hours of admission, you'll meet with our Nurse Practitioner for a comprehensive medical evaluation and medication management plan tailored specifically to you.

Individual therapy sessions happen weekly, where you and your therapist work one-on-one using Cognitive Behavioral Therapy to identify and reshape the thought patterns that fuel addiction. Dialectical Behavior Therapy builds your emotional regulation skills, giving you concrete strategies for the moments when clarity feels impossible to find.

Chapter II

Building Resilience

Once you can see clearly, the work turns to building the strength you'll need for the road ahead. Group therapy becomes your training ground — evidence-based skills groups in DBT, ACT, and relapse prevention give you practical tools that work in real situations, not just in a classroom.

Psychoeducation groups deepen your understanding of addiction and mental health. Motivational exercise groups build physical resilience alongside the mental. And because rigid curricula don't serve unique people, your treatment plan adapts to your preferences and progress — collaborative goal setting ensures you're building the resilience that matters to your life.

Chapter III

Restoring Connection

Addiction isolates. Recovery reconnects. Family therapy — available individually and through monthly Zoom support groups — helps repair relationships and gives your loved ones the education they need to support your journey. Fellowship time over lunch (provided daily for PHP clients) creates the bonds that become your recovery community.

Beyond the facility walls, sober activities expand your world: equine therapy at a local farm, group hikes through New Hampshire's trails, bowling outings, and community recovery events. These aren't just activities — they're practice at living a full, connected life without substances.

Chapter IV

Reclaiming Independence

The final chapter of PHP is about learning to stand on your own. Case management support covers the practical realities of rebuilding — resume writing, job search assistance, court paperwork, and connecting you with housing resources including sober living options.

Transition planning begins early, not as an afterthought. Your clinical team coordinates step-down to IOP or outpatient care, connects you with external therapists and psychiatrists, and enrolls you in our alumni program — a lifelong community of graduates who understand what you've been through. Your story at Trailhead may end, but the chapter you've started here keeps going.

Your Journey

How Your Story Unfolds

Dear You,
When you first arrive,

everything will be new — and that's okay. Our clinical team conducts a comprehensive assessment to understand your unique needs, history, and goals. Within 24 hours, you'll meet with our Nurse Practitioner for a medical evaluation. Together, we'll build a treatment plan that's yours — not a template, but a path designed specifically for where you are and where you want to go.

As the weeks progress,

you'll settle into a rhythm of growth. Daily skills groups in DBT, CBT, and ACT give you practical tools. Individual therapy deepens your self-understanding. You'll learn to sit with discomfort, regulate your emotions, and build the coping strategies that will carry you forward. This is where the real work happens — and where you'll surprise yourself with your own strength.

When you begin applying these skills,

recovery stops being theoretical. Sober activities — hiking, equine therapy, community events — become practice at living fully without substances. Family therapy helps rebuild the relationships that matter most. Fellowship with other clients reminds you that you're not alone in this chapter.

As your chapter at Trailhead draws to a close,

we don't just say goodbye. Your case manager coordinates the transition to IOP, outpatient care, or external support. You'll be connected with therapists, psychiatrists, and housing resources. And as a graduate, you join our alumni community — a network of people who understand, who celebrate your milestones, and who are always just a phone call away.

Most clients have some or all of PHP covered by insurance. We accept Anthem BCBS, Point 32 (Harvard Pilgrim + Tufts), Tricare, Uprise Health, and WellSense NH Medicaid. Affordable self-pay options and financial assistance are also available.

We verify your insurance before admission — it takes less than a minute. Call (857) 312-1697 or use our online form for a free, confidential verification.

PHP is our most intensive level — full days (9 AM-3:30 PM) with lunch provided. IOP is a step-down: shorter sessions (typically 3-3.5 hours) with morning, afternoon, or evening options. Both include individual therapy, group work, and medication management.

Absolutely. Individual family therapy sessions are available, and we host monthly family support groups via Zoom — open to any family member 18+, covering codependency, family systems, and how to support recovery effectively.

Your treatment team coordinates a step-down to IOP or outpatient care, connects you with external therapists and psychiatrists, and enrolls you in our alumni program for ongoing community support.

PHP is ideal for adults stepping down from detox or residential treatment, those actively struggling with substance use who need daily structure, or anyone whose clinical assessment indicates the need for our highest level of outpatient care.

Flexible scheduling (change sessions daily), family-style environment, staff with lived recovery experience plus professional credentials, 12:1 counselor caseloads, and same-day admissions. We adapt to your life instead of demanding you adapt to ours.
