Our IOP offers flexible AM, PM, and evening tracks — the ideal step-down from PHP or a standalone path for working professionals who need intensive support without full-day commitment.
Trailhead Treatment Center is Licensed & Accredited with Dual NH & MA Licensing and committed to Evidence-Based Care with accreditation in progress through JCAHO.
"The IOP evening track let me keep working while getting real help. I never thought I could balance recovery and my career, but Trailhead made it possible."
"Stepping down from PHP to IOP felt seamless. The same team, the same care — just a schedule that gave me more independence. Exactly what I needed."
"Having three track options meant my daughter could attend IOP around her college schedule. The flexibility made all the difference for our family."
IOP runs Monday through Thursday with three track options: Morning (9:00 AM – 12:30 PM), Afternoon (12:30 – 3:30 PM), and Evening (6:00 – 9:00 PM). Choose the track that fits your life — whether you're working full-time, attending school, or managing family responsibilities. Each 3 to 3.5 hour session delivers focused, evidence-based group and individual therapy.
The program typically lasts 60 to 90 days, with duration individualized based on your clinical progress and recovery goals. Your treatment team reviews your plan regularly and adjusts as you grow.
Within 72 hours of admission, you'll meet with our on-site Nurse Practitioner for a comprehensive medical evaluation and personalized medication management plan — including Suboxone, Vivitrol, or other FDA-approved medications as appropriate. Ongoing medication monitoring continues throughout your IOP journey.
Weekly individual therapy sessions pair you with a licensed clinician who works one-on-one using CBT, DBT, and ACT to reshape the thought patterns that drive addiction and mental health challenges. Your therapist coordinates closely with your entire care team.
Each IOP session includes group therapy, psychoeducation, and skills workshops rooted in DBT, ACT, and relapse prevention. Groups are capped at a 12:1 ratio, ensuring you get personal attention in every session. Snacks are provided during each track to keep you fueled and focused.
Holistic practices — breathwork, mindfulness, and guided meditation — complement the clinical curriculum. Because rigid approaches don't serve unique people, your treatment adapts to your preferences: 12-Step, SMART Recovery, Dharma Recovery, or a personalized blend.
IOP is ideal for adults stepping down from PHP who are ready for more independence, working professionals who need intensive treatment without leaving their jobs, and anyone whose clinical assessment indicates a structured outpatient level of care. The evening track is especially designed for people who work during the day.
Family therapy — available individually and through monthly Zoom support groups — helps repair relationships and gives your loved ones the tools to support your recovery. Fellowship time between sessions builds the peer connections that become your recovery network.
Case management covers the practical realities of rebuilding — resume writing, job search assistance, court paperwork, and connecting you with housing resources including sober living options. Transition planning begins early, not as an afterthought.
As you progress, your clinical team coordinates step-down to standard outpatient care, connects you with external therapists and psychiatrists, and enrolls you in our alumni program — a lifelong community of graduates who understand. Your time at Trailhead may end, but the support never does.
Morning, afternoon, or evening — choose the track that fits your schedule, and switch as your life requires.

Early IOP sessions focus on building routine and stability. Group therapy, psychoeducation, and your first individual sessions establish the foundation. You learn to balance treatment with daily responsibilities.
Skills groups in DBT, CBT, and ACT give you practical tools. Individual therapy deepens self-understanding. You apply coping strategies in real-world settings — work, family, and social situations — while your team monitors progress.
Your case manager coordinates step-down to standard outpatient care or external support. You're connected with therapists, psychiatrists, housing resources, and enrolled in our alumni community for lifelong support.
