Supporting someone who self-harms can feel overwhelming. Here are compassionate, evidence-based approaches.
Understand that self-harm is a coping mechanism, not attention-seeking. Learn why people self-harm and what function it serves so you can respond with empathy rather than panic.
Avoid reacting with shock, anger, or ultimatums about stopping. Express your concern calmly: “I care about you and I want to understand what you’re going through.”
Demanding that someone “just stop” self-harming can increase shame and secrecy. Instead, encourage professional help and offer to support them through the process.
Trailhead offers individual family therapy sessions and monthly Zoom education groups. Families learn about self-harm, how to respond supportively, and how to maintain healthy boundaries.
Self-harm serves a function — professional treatment helps find healthier alternatives. Offer to help research treatment options, make calls, or attend an initial appointment together.

Call for Family SupportIt’s common for people who self-harm to resist treatment. This doesn’t mean recovery is impossible — it means a different, compassionate approach may be needed.
Keep communication open without judgment. Let them know you care and that you’re available when they’re ready. Isolation makes self-harm worse.
Share information about treatment options, crisis lines, and support groups. Let them explore at their own pace rather than forcing immediate action.
Self-harm often helps manage overwhelming emotions. Acknowledging this without condoning the behavior shows empathy and builds trust.

Self-harm (also called non-suicidal self-injury or NSSI) refers to intentional injury to oneself without suicidal intent. Common forms include cutting, burning, hitting, hair pulling, and skin picking. While not a suicide attempt, self-harm is a sign that someone is struggling to cope with overwhelming emotions and needs professional support.
Not necessarily. Self-harm is often a way to cope with emotional pain, not an attempt to end one’s life. However, self-harm does increase the risk of suicide over time — that’s why professional treatment is important. At Trailhead, we take all self-harm seriously and conduct thorough safety assessments for every client.
Treatment length varies by individual need. Our PHP program typically lasts 4–8 weeks, IOP runs 8–16 weeks, and ongoing outpatient support continues with safety monitoring. Most clients benefit from a step-down approach — starting with higher intensity and gradually transitioning to less structured support as skills are strengthened.
Dialectical Behavior Therapy (DBT) is considered the gold standard for treating self-harm. Our program combines DBT skills groups (distress tolerance, emotion regulation, interpersonal effectiveness) with individual therapy and comprehensive safety planning for the most effective outcomes.
Absolutely. Our Evening Track (6–9 PM, Monday through Thursday) is specifically designed for working adults. We also offer afternoon IOP sessions and telehealth options. You can switch between time slots daily based on your work schedule.
Most major insurance plans cover self-harm treatment as a mental health condition. We are in-network with Anthem BCBS, Point 32 Health (Harvard Pilgrim + Tufts), Tricare, Uprise Health, and WellSense (NH Medicaid). Our admissions team can verify your benefits in minutes — call (857) 312-1697 or use our online form.
Self-harm often co-occurs with trauma. We use trauma-informed approaches throughout our programming and can coordinate with trauma specialists when clinically appropriate. Trauma processing is introduced once safety and coping skills are firmly established.
Yes — family involvement is encouraged. We offer individual family therapy sessions coordinated with your primary therapist, plus monthly family education groups via Zoom. Families learn about self-harm, how to respond supportively, and how to maintain healthy boundaries. Open to any family member 18+.
Relapse prevention is built into every treatment plan. If urges return, you have access to your safety plan, crisis resources (988 Lifeline, Crisis Text Line), our alumni program, and ongoing outpatient support. Recovery is a process, and we provide long-term tools and support.
Our differentiators include: a DBT-focused, safety-first approach, a compassionate and non-judgmental environment where staff know every client by name, flexible scheduling across morning/afternoon/evening tracks, 12:1 counselor caseloads for personalized attention, and same-day admission availability when clinically appropriate.
Your recovery begins with a single phone call. We’ll handle the rest with compassion and care.
